An English language certificate is not required to take the IMAT exam. The IMAT (International Medical Admissions Test) is conducted in English, and your ability to understand and answer questions in English is implicitly tested through the exam. If you do have, though, a certificate you may add it in the IMAT exam application and this will you benefit in case of same scores with another candidate.
